2012 REGISTRATION: Registration available at Culver Insurance, 31 Grant Ave Ext, Monday- Friday or Download registration forms under the Forms link, click on Medical Forms. There are 2 forms to be completed and signed. Mail forms to P.O. Box 374, Penn Yan ** $60/child or $95/family which includes the fundraising fee ($30/single and $45/family)which is refundable after raffle tickets are sold. Checks payable to: PYYLL
